Meet Kamal Young, a four-year member of EOCP’s Housing Opportunities for Persons with AIDS (HOPWA) team who works as a Medical Case Manager for a vulnerable group of clients. Since the late 1990’s, EOCP has served individuals and families living with HIV/AIDS through 25 dedicated shelter beds at our Crossroads shelter. Our team is composed of an onsite Medical Case Manager, who works alongside our Outreach Worker and our Case Management Services Manager. The team is cohesive in making contact, serving and providing aftercare for people living with HIV/AIDS in Alameda County.

Outreach, a key component of the services in the community, includes collaborations with hospitals, clinics, and county jails.  Outreach also includes interacting with unhoused people and offering them access to shelter. Once a client is ready for an intake, they will be assessed to create a housing stability plan and introduced to other intensive case management services. Each client is also provided an assessment by our Substance Abuse Counselor. Follow up begins weekly. Clients are given a Coordinated Entry Services assessment within the first two weeks so they can be placed on the Alameda County community list for permanent supportive housing matches.

EOCP’s HOPWA team provides each client with all the resources at the program's disposal for both onsite and aftercare services. Clients who have been placed in permanent housing are provided with after-care case management for up to six months. Sometimes, eligible clients will go directly into our aftercare case management services if their housing is coming to an end or are unable to stay in the shelter.  As a team, we work very closely on client progress and we support each client so they can achieve the best outcomes.
